JMIR Public Health and Surveillance - Symptom Trajectories and Clinical Subtypes in Post–COVID-19 Condition: Systematic Review and Clustering Analysis

> Network analysis showed that core post–COVID-19 symptoms evolved from early respiratory-neurological manifestations to chronic multisystem symptoms dominated by dizziness. Clustering analysis further indicated a progressive convergence of 2 initially distinct post–COVID-19 subtypes, with the acute inflammatory type becoming less prominent and gradually transitioning into a more chronic, persistent pattern. ... The results reveal a progressive decline in respiratory symptoms over time, while neurological manifestations emerge as the most persistent and systemically impactful core symptoms.

JMIR Public Health and SurveillanceSymptom Trajectories and Clinical Subtypes in Post–COVID-19 Condition: Systematic Review and Clustering AnalysisBackground: Lack of specific understanding of clinical characteristics, disease trend, and key symptoms of post COVID-19 condition severely impacts research and discovery of new preventive and therapeutics drugs. Objective: This study aims to explore the prevalence, trends, and interrelationships of clinical symptoms at different follow-up time points in post COVID-19 condition. Methods: The PubMed, Web of Science and Embase databases were searched from December, 2019 to April, 2024. Observational studies related to the prevalence of symptoms in post COVID-19 condition patients had been included in this study. We conducted a meta-analysis of symptom prevalence following PRISMA guidelines, and used network and clustering analysis to explore the relationships between symptoms. Results: This analysis incorporated 137 sets of macro-level data from 99 clinical studies, involving a total of 45,427 patients. The most common symptom among patients from all 4 time points was fatigue. Dyspnea ranked second in symptom prevalence during both the 3rd and 6th follow-ups, and its prevalence consistently declined subsequently. Additionally, we found significant differences in symptom prevalence across different geographical regions and correlations among symptoms, especially between cough, dizziness, and other symptoms. The study also identified that post COVID-19 condition patients can be categorized into mild and severe based on symptom presentation. Conclusions: With varying follow-ups and geographical locations, the number and prevalence of symptoms vary. However, neurological and respiratory symptoms remain the predominant manifestations. Cough, dizziness, palpitation, and loss of smell may serve as key symptoms aiding in the diagnosis and differentiation of post COVID-19 condition. Further research is warranted to elucidate underlying mechanisms, refine diagnostic criteria, and develop targeted interventions. Clinical Trial: PROSPERO Number CRD42024537825

This paper nature.com/articles/s42255-025 is interesting. They tested Vitamin B3 in a pill that is

designed to release nicotinamide [a form of Vitamin B3] in the lower small intestine and colon

And found:

1️⃣ Acute COVID-19 might be safely and easily reduced in severity and duration
2️⃣ In general, no improvement in Long COVID rates (here termed Post-COVID syndrome, PCS)
3️⃣ BUT, for people with recovery-like trajectories, they found superior outcomes. This takes a few quotes:

The subgroup was

participants at risk who had shown improvement in the primary endpoint or one of the three key secondary endpoints in the acute phase of the disease

In [this] subgroup, a significant benefit of nicotinamide was also observed in participants with PCS (nicotinamide: n = 48, PCS score 8.33 ± 0.84; placebo: n = 57, PCS score 11.82 ± 1.03; absolute difference: −3.49; 95% confidence interval, −6.1 to −0.86; P = 0.010

Now we all know how Long COVID isn't one single disease. Let's assume for a second that other results that suggest at least one subset has a microbiome component are correct.

As they say "life finds a way" so it would make sense that remnant gut populations could rebound over time and that this subset of LC sufferers might be more likely to have recovery trajectories. (This while we have no treatments for any subset.)

This obviously won't stop the pandemic nor its consequences, but still, here's hoping that there's no patent encumbrance on coating B3 for lower-gut delivery.

Of course here in BC we'll get access the same time we get access to Metformin. ⌛

NatureNicotinamide modulates gut microbial metabolic potential and accelerates recovery in mild-to-moderate COVID-19 - Nature MetabolismIn a prospective, double-blind, randomized, placebo-controlled trial, supplementation with a combination of conventional and gut-targeted nicotinamide was safe and effective in ameliorating COVID-19 symptoms.

Not excactly a good thing that I slipped on my way out of the bath tub yesterday night, but with that as the scenario:
1. We own crutches ourselves.
2. I did not hit my head, the #pcs (Post Concussion Syndrome) makes me ridicilously afraid of head setbacks.
3. I have the very best husband, who spoils me with bedside coffee. 🥰
And even 4.!
It is the left foot that hurts, so I will be back in the car in a few days. No clutch in our "new" car.

"New York Times bestselling author Cory Doctorow returns to the world of Red Team Blues to bring us the origin story of Martin Hench and the most powerful new tool for crime ever invented: the personal computer.

The year is 1986. The city is San Francisco. Here, Martin Hench will invent the forensic accountant — what a bounty hunter is to people, he is to money — but for now he’s an MIT dropout odd-jobbing his way around a city still reeling from the invention of a revolutionary new technology that will change everything about crime forever, one we now take completely for granted.

'Picks And Shovels' is a high-tech crime thriller about the “weird era of PCs” in the early 1980s, when Reaganomics, the tech industry, multilevel marketing, and the AIDS crisis all converged in San Francisco. "—Jacobin

The Guardian and BBC are both leading on this story this morning.
Once again we have a broad brush statement from politicians that civil service performance needs improving without any real evidence.

Performance related pay seems to be on the cards again, my experience of it was not good. Hours spent collecting and documenting personal evidence as well as reviewing staff's evidence. Then an annual series of comparison meetings to decide where people fitted on the arbitrary bell shaped curve that determined who got what. The individual bonuses involved were not much, IIRC the year I got a top award the amount was around £400.
I really hope this doesn't return as it was a waste of time, a divisive exercise for no real gain for anyone.

To improve performance they need to:
Drop competency based criteria and bring back minimal qualifications for new starters.
Improve pay conditions.

ING tegen beroofde klant: fuck you

Seriously, #Apple preventing people and businesses from #refurbishing should be illegal!

youtube.com/watch?v=wfwy-y5rCo

And I sincerely hope both @EU_Commission and @Bundesverband will actively go after manufacturers that want to deny people their freedom to actually own their devices as per "1st sale doctrine".

Like it's none of Apple's business if someone chose to get a #refurbished display unless they want to fraudulently sell it as a new device and not a replacement - but that's already covered with fraud.

Why should the #ONNDP wait upon the sitting of #ONLeg? They could have acted immediately, they can act now.

#MPP Jama - forbidden to speak or be recognized in the chamber, ejected from caucus by ONNDP leader Stiles - has managed to raise an issue important to her #constituents and confound the #ONPCs from the independent seats.

Meanwhile, Stiles tries to ride Jama's coattails, but will give the Speaker, Ford, and #PCs the spotlight and (another) free shot.

Startups are raking in up to $85,000 per day by recycling #gold and #copper from #electronics thrown in the trash — #EWaste 'gold mining' efforts are expanding

By Aaron Klotz
published January 31, 2024

The e-waste recycling boom is upon us.

"Did you know that the materials inside your old, outdated TV, laptop, desktop, and other electronic devices contain precious metals such as gold and copper? According to Business Insider, small startups are cashing in on the untapped potential of the e-waste industry, making as much as $85,000 per day #recycling old electronic #CircuitBoards. The initiatives will also likely continue to expand — worldwide, there's a projected $55-$60 billion worth of precious metals inside abandoned circuit boards just waiting to be collected.

"E-waste is quickly becoming one of the biggest #environmental #hazards affecting our world today. More than 50 million tons of electronics get tossed in the trash, with much of that garbage being sent to third-world countries (most notably #India) for recycling. It's an environmental nightmare that's projected to become substantially worse over the next decade due to our insatiable desire for new #phones, #laptops, #gaming #PCs, #TVs, and anything that requires electricity to run.

"On top of this, much of our e-waste doesn't get recycled at all, being left to rot in garages, #landfills, and city streets. The United States alone only collects about 15% of all the e-waste for recycling that its citizens create on a daily basis.

"The flip side to this situation is that e-waste is simultaneously becoming one of the most underrated 'gold mines' of the modern era, thanks to all of the valuable metals housed inside most circuit boards. Worldwide, there's a projected $55-$60 billion worth of precious metals inside abandoned circuit boards just waiting to be collected.

"Business Insider followed several scrappers and a small startup in Sydney, #Australia, to see how much income these smaller entities can make from recycling e-waste. It found that solo scrappers can't make a full-time income out of e-waste yet, but scrappers who work in larger organizations, particularly organizations / startups with access to heavy machinery, can generate some serious income.

"One such startup that Business Insider interviewed was #MintInnovation. Scrappers who find e-waste and drop it off at Mint Innovation can make as much as several thousand dollars per load. Mint Innovation has virtually mastered the art of metal extraction from e-waste, utilizing tons of special machines and a secret special sauce designed to break down electronic circuit boards and separate the valuable metals inside from unwanted plastics and other materials the metals are attached to.

"The fully automated setup Mint Innovation has built can generate an impressive $85,000 of income per day from the raw metals it collects from e-waste. That translates into about $30 million in income per year.

"Mint Innovation perfectly demonstrates the amount of potential income the e-waste industry can access right now. The problem, however, is that there is not a lot of infrastructure dedicated to e-waste processing, making it difficult for e-waste scavengers to make a livable income. That could easily change in the future, as e-waste pollution continues to rise and more entrepreneurs understand the income potential e-waste can generate.

"If you want to take a crack at recycling your own e-waste, we made a fun tutorial several years ago to teach you the basics.

#Windows10 end of life could prompt torrent of e-waste as 240 million devices set for scrapheap

By Ross Kelly
published December 21, 2023

"As Windows 10 end of life approaches, analysts are concerned that millions of devices will be scrapped due to incompatibility

"Analysts have warned Windows 10 end of life plans could spark a global torrent of e-waste, with millions of devices expected to be scrapped in the coming years.

"Research from Canalys shows that up to 240 million #PCs globally could be terminated as a result of the shift over to Windows 11, raising critical questions about device refreshes and the responsibility of vendors to extend life cycles.

"A key concern highlighted by Canalys was Microsoft’s current practices regarding Windows 11 device compatibility.

"Minimum system requirements for installing Windows 11 on a PC mean users must have a processor of at least 1 GHz or faster along with a minimum of 4GB RAM. Storage requirements are also set to a minimum of 64GB.

"Many of the devices set for scrapping after the Windows 10 support deadline, set for October 2025, won’t meet these requirements despite being in 'good condition' and potentially #recycled [or repaired] by enterprises.

"The consultancy added that many of these 240 million devices will end up in #landfill."

"High risk" has been twisted to focus exclusively on the potential for "severe" acute COVID (aka hospitalization *for*), ICU stays, and mortality, entirely omitting the high risk of morbidity conferred by reinfection after reinfection: organ and other compounding systemic damage, newly-developed cancer / autoimmune disease / chronic illness, and any other sequelae that slip through the diagnostic cracks.
