I am convinced that the phrase "root cause" and it's loftier sister "RCA" (root cause analysis) have become semiotic objects that have grown past their raw definitions.
We are using them like this:
OH: "I haven't used Google in my browser search for a year but I still use the word 'google it' to mean search the web."
OH: "The RCA ticket is opened at the beginning of the incident even though we have no idea that a RCA can be run."
